What to send me for your personal return
 
Here are some reminders about what to send at year end (if possible, keep like-items together. Ex: Interest statements):
Personal return:
1 New dependent
Full name
social security number
birth date
2 If you have moved
Date moved
New address
County you are now in
City limits you are now in
3 W-2s
4 1099s
5 K-1s
Any information you received from the partnership/estate/trust
6 Stocks and investments
All of the forms your broker sends to you
If you've sold stock, send your purchase information, if you have it
7 Home purchase or sale
Settlement statement
8 Child care
Provider's name
Provider's address
Provider's tax id number (ein or social security number)
9 Medical expenses
This depends on circumstances on each return.
Normally taxpayers do not have enough to be of benefit on their return. If, however,
you have incurred a lot in medical expenses, let me know.
10 Mortgage interest on principal residence and vacation home
Send even if we do not itemize on your return
11 Real estate taxes on principal residence and vacation home
Send even if we do not itemize on your return
12 Charitable contributions
Cash/Check: An itemized list is best
Non-cash: Date of contribution
Name and address of charity
type of items (household goods, clothing, furniture, etc)
value (what one might pay in a thrift shop)
13 Employee business expenses
Expenses by category
Indicate any employer reimbursement on the expenses you've listed
14 State information
If you live in a State other than Ohio, please send your entire package
15 City returns
Entire book
16 School District tax (if it applies)
I do not need the entire book, but do include the preprinted envelope
